Defeat it and Diablo 4 Items loot its head.

6. Offer the Head to Hluttred's Spirit

Place the Fallen's Head into the bowl near Hluttred's shrine to begin the ritual.

7. Stand in the Horadric Glyph

Step into the glowing Horadric Glyph circle.
Wait and observe the ritual.

8. Survive the Ambush

A large demonic ambush begins after the ritual.
Survive waves of enemies - ideally using AoE-heavy builds or crowd control spells.

After the ambush, exit the cave and meet Bryona just outside.
This concludes the quest.
